AccuZIP6 had a problem writing a header record. Give this a try, which should solve the error:
With your database open, pull down the File menu to Export.
Choose all the fields you need, select CSV as the export format, and click Export. Give the new file a name.
After saving this file, close your database, then pull down File to Open List.
Open that new csv file. Assign field names to the data, then import it.
Validate the database, and that should get rid of the error.
